***If you do not see your insurance listed, please call our office to check.

As a courtesy to you, we are happy to handle the submission of claims to your insurance company and accept reimbursement directly from them. However, your estimated co-pay, including any deductible, is due the day services are rendered. Any amount that is left unpaid by your insurance or dental claim that is not paid within 60 days is your responsibility. In the case of an account default, you are responsible for any additional collection charges that may be applied to your account, including but not limited to finance charges (18%), outside collection service fees and reasonable attorney fees. Also, a charge of $25 plus any incurred bank charges will apply to checks that are returned for any reason.

Co-Pay Estimates

Your insurance benefit plan is a contract between you and the insurance company. Although we do our best to help you understand your insurance coverage, we are unable to provide a guarantee of what your insurance will reimburse our office on your behalf. We also will not allow insurance benefits to dictate what treatment is best for your child. Should you have specific questions regarding your plan's limitations, we will try our best to answer them, or will provide the phone number to contact your insurance carrier directly.
